Pronóstico del pie diabético basado en la insuficiencia vascular periférica de pacientes tratados en el hospital regional de Ica 2023 a 2024

Resumen
Objetivo. Determinar si la insuficiencia vascular periférica es un factor pronóstico para
amputación del pie diabético en pacientes tratados en el Hospital Regional de Ica 2023 al 2024.
Metodología. De tipo observacional, trasversal, retrospectiva y analítica, de nivel explicativo.
Datos que fueron obtenidos de las historias clínicas y analizadas em el programa estadístico
SPSS v29, y analizadas de manera bivariado y multivariada con 95% de confianza. Resultados:
Los factores pronósticos para amputación del pie diabético com insuficiencia vascular son:
Edad de 60 a más años p=0,007 ORa=2,5 (IC95%:1,3-4,9), sexo masculino p=0,012 ORa=2,5
(IC95%:1,2-5,1). Tener un mal control de glicemia p=0,000 ORa=4,6 (IC95%:1,9-10,6), tiempo
de evolución de 10 años a más p=0,003 ORa=2,8 (IC95%:1,4-5,5), estar en grado 3 a 5 de la
escala de Wagner p=0,000 ORa=5,4 (IC95%:2,7-10,9). Tener hipertensión arterial p=0,003
ORa=3,4 (IC95%:1,5-7,5), bajo peso (IMC < 18,5) p=0,000 ORa=4,9 (IC95%:2,4-10,2).
Consumir tabaco p=0,002 ORa=3,6 (IC95%:1,6-8,1), consumo de alcohol p=0,006 ORa=3,1
(IC95%:1,4-6,9). Conclusión: La edad de 60 a más años, sexo masculino el mal control de la
glucemia, tiempo de enfermedad de 10 años a más, el estadio 3 a 5 en la escala de Wagner la
hipertensión arterial, el bajo peso el consumo de tabaco y alcohol son factores pronósticos para
amputación del pie diabético
Objective: To determine whether peripheral vascular insufficiency is a prognostic factor for diabetic foot amputation in patients treated at the Regional Hospital of Ica from 2023 to 2024. Methodology: Observational, cross-sectional, retrospective, and analytical study with an explanatory level. Data were obtained from medical records and analyzed using the SPSS v29 statistical program. Bivariate and multivariate analyses were conducted with a 95% confidence level. Results: The prognostic factors for diabetic foot amputation with vascular insufficiency are: age 60 years or older (p=0.007, ORa=2.5, 95% CI: 1.3-4.9), male sex (p=0.012, ORa=2.5, 95% CI: 1.2-5.1), poor glycemic control (p=0.000, ORa=4.6, 95% CI: 1.9-10.6), disease duration of 10 years or more (p=0.003, ORa=2.8, 95% CI: 1.4-5.5), Wagner scale grade 3 to 5 (p=0.000, ORa=5.4, 95% CI: 2.7-10.9), arterial hypertension (p=0.003, ORa=3.4, 95% CI: 1.5-7.5), underweight (BMI < 18.5) (p=0.000, ORa=4.9, 95% CI: 2.4-10.2), tobacco use (p=0.002, ORa=3.6, 95% CI: 1.6-8.1), and alcohol consumption (p=0.006, ORa=3.1, 95% CI: 1.4-6.9). Conclusion: Age 60 years or older, male sex, poor glycemic control, disease duration of 10 years or more, Wagner scale grade 3 to 5, arterial hypertension, underweight, tobacco use, and alcohol consumption are prognostic factors for diabetic foot amputation.
